I understand the budget payment is subject to change based upon annual usage. I also understand that this is not an extended pay program. Deliveries will not be made if payments are delinquent. |

| FAST 4 DISCOUNT PLAN: Subject to credit approval, you will be mailed an invoice on the next business day after your fuel dlivery is made. To get a 4 cent per gallon discount, just pay the discounted amount shown on your bill within ten days of delivery. | ||
| DELIVERY AUTHORITY: The customer agrees to accept the meter-printed delivery ticket (without signature) as proof of delivery- unless other arrangements are made in writing with Globe Petroleum. | ||
| PRICE: Customer understands that the price per gallon of oil is subject to change daily with market fluctuations, Customer agrees to pay Globe Petroleum's prevailing price per gallon on the date of delivery. Customer may elect to enroll in our Price Cap Program each year. See other application. | ||

| GLOBE PETROLEUM OIL CREDIT POLICY
I acknowledge the terms offered by Globe Petroleum. I agree to pay interest at a rate of 1 1/2% per month for all invoices past due. An additonal 24% wil be charged for attorney's fees, in the event that a claim for non-payment of any balance due that is referred to attorney for collection. The below signatures also grants Globe Petroleum the right to check any factors pertinent to a fair evaluation of establishing credit. |
